STUDENT ADMISSION
New student information is required for each child interested in Seacrest Christian School. Please call or visit the School so we may check availability of classes and provide registration information. A formal packet of information is available by calling the School office at (561) 276-5552 or by e-mailing the School at school_info@seacrestchurch.com. We welcome your call or email.

Health and Immunization
Who Needs a School Physical?
School physicals are documented on Florida Department of Health Form 3040 provided by your child's Physician. TB testing is no longer mandatory but is at the discretion of the physician based on a student's risk of exposure.

Original Immunization Form (K5, 1st, and new to the school) or appointment card

Original Physical Form (K5, 1st, and new to the school) or appointment card

1st through 5th Graders' Report Card (copy of last report card)
Reminder: All of the above information must be returned to the School Office before your child is placed on a class list. |
